3 page version - This is the compact version; you cut it into 13 quarter-pages and it can easily fit into a pocket.

Large print version - In 16-point font for ease of reading, but it's 10 full-size pages long.

GC - good condition
BS - both sides
WP - working properly
WS - well secure
SBE - secure at both ends
Gr - greased
D - damage
C - cracks
L - leaks
MB - missing bolts
B - bends
IW - illegal welds

A note on the order of the list: My notes differ in certain places from the order of components that some consider the best practice. This is the script I prefer - it makes sense to me to group all brake components together for each section, and all suspension components. By some reckoning this is “jumping around” because the components, especially on section B, are not always adjacent to each other. I also group everything on the in-cab inspection that you can have the key off for, then everything you need the key on for, and then everything you turn the engine on for.
