Employees when returning to work has to submit a proper return to work to the office. The “return to work” forms are usually provided by the organization which they work for. The form displays the name of the organization at the top and includes certain fields below for the employees to fill in. It basically involves two parts. The first part is a self certification by the employee wherein the actual reason for the leave of absence is mentioned. The reasons may vary from being sick to any emergency that they have to handle personally.

In the self-certification part, the employees may have to provide the details like date, job title, day(s) of absence, return to work date, reason for absence, name of the authority that they report to and signature of the employee.
