The Czech Republic Passenger Locator form is an international travel document that collects information from international visitors to the Czech Republic. This document will aid track any potential illness entering the country through personal inspection. Please, think of it as your "IMMERSE" travel guide, only this one time! Remember, it doesn't replace any travel documentation.

The purpose of the Czech Republic passenger locator form is to collect detailed information from travelers arriving into the Czech Republic for both personal and tourist purposes. First, your name, complete birthdate, current mailing address, and contact number if any, passport type (national and international), and country of origin. If you have a different country of origin, please use the correct translation (please do not confuse!) ), and then, after your name and country, type your arrival address. If you are travelling within the EU, the second item in the row will be for your destination country, please see the table below.

The second item on the list of questions is a short explanation about your need for a passport. If you're traveling between the EU and the USA or Canada, you'll need a visa to enter the country. This is commonly known as a COVA (or visa card). You must be aware that without a visa you will not be allowed entry at all! You must fill out the Czech Republic passenger locator form accordingly.
