Elena Merchand is a healthcare professional in every sense of the word. She began her career as a certified wound care nurse, after she received her certification from the Wound Care Education Institute. She also became a certified in diabetic wound care and worked closely with senior patients for several years.

Elena took note of the problems that her patients were facing the realm of senior real estate and transitional care, and vowed to help create a solution. She was part owner of Pathways to Care, which was a business that creates a “one stop shop” for senior care solutions. They provide financial planning and guidance for long term care.

Elena then went on to join the ranks of HomeHero, which is a tech startup dedicated to helping seniors and their caregivers in California (and beyond) find the most affordable at-home care. She was the Vice President of field marketing, and is excited to bring this service into the lives of the seniors who need it.

Most recently, Elena is the owner of Senior Home Advocates. This is a company that works to help physicians and health care providers find value in their practice through Transitional Care Management and Chronic Care Management.

Elena is deeply passionate about the care of maturing adults, and is always looking for another way that she can help.
