About JMK Medication Consultants
JMK Medication Consultants are an independently owed and operated pharmacy consulting service. Specializing in geriatric medicine and poly-pharmacy.
The Home Medicines Review (HMR) and Residential Medication Management Review (RMMR) are programs funded by the Australian Government (Item 900 on the Pharmaceutical Benefits Scheme PBS). They are designed to assist individuals to maximize the benefits of their medication and reduce or prevent medication-related problems.
HMR and RMMR's can only be conducted by experience and qualified pharmacists who have undergone accreditation by the Australian Association of Consultant Pharmacists (AACP).
JMK Medication Consultants are dedicated to maintain the highest professional standards. They are determined to ensure:

For HMR referrals by a General Practitioner and/or Community Pharmacists:
General Practioners (GP) or Community pharmacists that require a AACP accredited pharmacist to conduct HMRs/RMMRs can contact JMK Medication Consultants for immediate referrals.

How to refer?
Referrals should include:
- Patient contact details
- Medical history
- Current medication list
- Relevant notes highlighting main concerns and reason for referral
- Relevant pathology (Blood tests, U & E results)
Referral Procedure
- Identify a patient who would benefit from a HMR
- A referral can be generated & sent directly to JMK Medication Consultants ([email protected])
- Referrals can be in a the form of a simple letter or via templates found on prescribing software.
- Once a referral has been received JMK Medication Consultants will contact the patient directly to arrange an appropriate time for conducting a HMR.
- Upon completion of HMR, a HMR letter will be composed and sent directly to referring GP and local pharmacy highlighting f
