Maria D'Souza, MD MPH

Geriatrician in Wilmington, Delaware

Greetings! I am a geriatric medicine physician with 10+ years of experience providing holistic medical care for older adults in a variety of settings and contexts, including primary care, nursing facilities, assisted living, memory care, house calls, and hospice and palliative care. I have also served as the Medical Director of a PACE program (Program of All-inclusive Care for the Elderly), enabling older adults with complex medical needs to continue living in their own homes.

My patients often tell me how much they value the way that I listen. Each older adult has a unique story that I am passionate about hearing and understanding in order to provide true person-centered care. Discussing individual values and goals allows us to formulate a plan of care together and to partner with all members of the care team in achieving those goals. Geriatrics is a team sport!

Contact me to book a consultation, join my email list or schedule a presentation to your group about health and wellness issues affecting older adults, including:

* identifying priorities and individualizing goals for health and wellness

* brain health and memory concerns

* elements of comprehensive dementia care

* preventing falls and fractures

* caring for the caregiver / meditation...and more

I look forward to connecting with you and learning how I might be able to help!
