Paul Smith

As the owner of Extended Family Associates, Paul R. Smith, RN, puts his three decades of experience as a Registered Nurse and his 18 years of experience as a case manager to good use. Extended Family Associates is an elder care firm that strives to keep family members at home, even into old age. Paul R. Smith, RN, and his partner, Jude Moreno, a Certified Nursing Assistant and Certified Home Health Aide, offer comprehensive geriatric care management. At the outset, each patient is screened and all health problems are identified. The company then considers the patient's resources and ultimate goals in order to provide plausible solutions. Additionally, the team will help patients plan financially and proactively to ensure a smooth future.

Paul R. Smith, RN, also utilizes a large network of attorneys, accountants, counselors, financial planners, caregivers, and physicians in order to make appropriate referrals as needed. Other services available at Extended Family Associates include medication management assistance, housework, shopping, day care, respite care, long-term placement, and support for long-distance families. Supplementing his professional abilities, Paul R. Smith, RN, possesses membership with the National Association of Professional Geriatric Care Managers, a non-profit group dedicated to advancing the profession of geriatric care.

Before he launched his own company in 2007, Paul R. Smith, RN, served as a Case Manager with Gentiva Health Services in San Luis Obispo, California. There, he managed between 11 and 15 homebound patients, offering direct nursing care, completing assessments, and implementing care plans. He also coordinated and communicated with physical therapists, occupational therapists, speech therapists, and physicians, while supervising ancillary staff.

Smith earned a Bachelor of Arts in History and Political Science from Marquette University and continued his education at Creighton University, where he earned his Bachelor of Science in Nursing. Finally, Paul R. Smith, RN, returned to school one more time to obtain his Master of Business Administration from Keller Graduate School of Management.
