Stacey McNeely
National Director Transitional Care Coordination in Minneapolis, MN
A graduate of the University of Minnesota, Stacy McNeely holds a bachelor of science in marketing and management and a Registered Nurse degree. In her most recent role as National Director of Transitional Care Coordination for The Goodman Group in Chaska, Minnesota, Stacy McNeely combined her business expertise and clinical experience to create Able Care Connect. The program utilizes TCM codes as a billable service through Medicare and helps identify gaps in patient care and reduces rates of hospital readmits.
Ms. McNeely also created Dreamcatcher, a nonprofit that offers scholarship opportunities to retired military members. Previously, she served as a Device Nurse with the University of Minnesota Physicians Heart Clinic, where she cared for patients with cardiac arrhythmias or implanted cardiac devices.
In the 1990s, Ms. McNeely competed as a professional barrel racer in the Women's Professional Rodeo Association and the International Professional Rodeo Association. She earned several top 12 finishes. She currently supports animal rescue organizations including Can-Do Canines and Underdog Rescue.
